NCQA Recognition – Patient-Centered Medical Home

Patient-centered models are taking hold in communities across the country to achieve what healthcare reform movement refers to as the Triple Aim:

  1. Improve the health of the population,
  2. Enhance patient experience of care with better quality, access, and reliability, and
  3. Control healthcare costs.

Both offices of Westfield Family Physicians have received the highest level (level 3) recognition as a Patient-Centered Medical Home™ (PCMH) by the National Committee for Quality Assurance (NCQA).

We are continually developing the disease management and health maintenance tools in our computer system, to deliver evidence-based care to patients with chronic conditions such as diabetes and hypertension, in an effort to truly make a difference in the health and well-being of our patients.
