Patients dealing with chronic disease are supported by our providers and staff in several ways. We offer education in many forms including small classes, individual instruction, printed materials and on line information. Referrals are made to Certified Diabetic Educators or dietitians as needed to help our patients manage their disease. Our electronic record enables us to track our patients’ progress and reach out to those needing services or case management as needed or desired. We strive for a team approach in helping patients manage their particular condition.
