Management Program

Chronic Disease Self Management Program

Developed at Stanford University. 2 ½ hours per session, 1 session per week for 6 weeks. The program is designed for those with type 2 diabetes or for those who are close to someone with type 2 diabetes. Subjects taught include:

  • Techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration
  • Appropriate exercise for maintaining and improving strength and endurance
  • Healthy eating
  • Appropriate use of medication
  • Working more effectively with health care providers
  • Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program.

Stepping On

Goal of the program is to build confidence and reduce falls. Stepping On is research based and scientifically proven to reduce falls! 2 hour sessions, 1 session per week for 7 weeks. A range of topics is covered all with the common theme of reducing falls and increasing self-confidence. With knowledge, you can be in control of falls.

  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Pacing activity and rest
  • How to evaluate new treatments.

Developed by the National Council on Aging, the Aging Mastery Program was developed as a guide for helping to navigate longer lives and make the most of retirement years. If you are recently retired or planning to retire soon this class is for you! A variety of topics are reviewed: exercise, sleep, healthy eating and hydration, financial fitness, advance planning, healthy relationships, medication management, falls prevention, and community engagement.
